Main findings of this research are facilitators including strong leadership and support from different stakeholders, elaborated, clear and well-structured and concise models about the interventions, adequate source and funding, participation of service users and communication among different agencies and their collaboration. This research has identified some barriers like lack of source, poor communication, lack of evidence based interventions.
